Prerequisites	The prerequisites for entering a PowerPath training are mandatory. Please check the box below to confirm that prerequisites will be completed prior to attending the first day of Level I training. I confirm that prerequisites will be completed prior to attending the first day of Level I training. Prerequisites include: Basic Certification Training Reading the User’s Guide. Conducting two Basic Screenings (using the information listed in Part III of the User’s Guide) - one on a colleague or family member the other on a program participant. Filling in the Personal Profile Folder and the Basic Response Booklet. Entering data into the PowerPath software and generating a Basic Report for the participant. Bringing completed forms, the Basic Report and your questions to the training. Update Training Reading any materials sent to you prior to the training.
